MK-677 shows up on every "best peptides" list on the internet, including one I was just handed. It isn't a peptide, and its own developer's trial data is a big part of why it never became a drug.
What MK-677 is
MK-677 (ibutamoren) is a small molecule, a non-peptide oral compound, not a chain of amino acids. It binds the ghrelin receptor (GHSR-1a) as an agonist, stimulating pulsatile growth hormone secretion and raising IGF-1 levels. It's grouped with research peptides online because it targets the same growth hormone axis, not because it's chemically a peptide.
What's actually been studied
- Older adults. A 2-year, double-blind, randomized, placebo-controlled trial in adults aged 60 to 81 examined whether MK-677 restored growth hormone and IGF-1 levels to those seen in younger adults. It did raise GH and IGF-1 and increased fat-free mass, but did not improve measured functional strength or clinical outcomes (Annals of Internal Medicine, PubMed).
- Kidney disease population. A randomized, blinded study in hemodialysis patients examined the oral ghrelin receptor agonist MK-0677 and found it increased serum IGF-1 in that population (PubMed).
- Safety signals that mattered. In an elderly hip-fracture population, a related trial was terminated early after safety signals including elevated fasting glucose, increased HbA1c, and congestive heart failure events.
Where the evidence is thin
The core finding across the strongest human trial (the 2-year older-adult study) is a real disconnect: MK-677 reliably raised GH and IGF-1 and increased fat-free mass, but did not translate into improved strength or functional outcomes, which is what would actually matter for most of the reasons people discuss it online. Combined with the safety signals in a frailer elderly population, this is why Merck's own development program didn't proceed to an approved drug.
The regulatory picture
MK-677 is not FDA-approved for any indication. Merck's investigational new drug program was discontinued following its pivotal trial results, and no New Drug Application was ever submitted. It's sold by research chemical vendors, though again, not actually as a peptide, strictly for laboratory research, not for human use.
Papa's take
This is a compound where the developer's own two-year, well-designed trial is the best evidence available, and that trial's actual finding, GH and IGF-1 go up, fat-free mass goes up a bit, functional strength doesn't improve, gets routinely dropped from online summaries in favor of just the first part. The safety signals in the frailer elderly trial are also worth knowing about, not just the headline mechanism.
