Semax is one of the few peptides on this list with a real body of published human clinical data behind it. Like Selank, that data comes almost entirely from one country's research network, which matters for how it should be read.
What Semax is
Semax is a synthetic peptide derived from a fragment of adrenocorticotropic hormone (ACTH), developed in Russia and studied as a nootropic (cognitive-enhancing) and neuroprotective compound, administered intranasally in the research literature.
What's actually been studied
- Stroke recovery. A study of 110 post-stroke patients compared early versus late rehabilitation timing, with intranasal Semax given in both arms, and reported increased plasma brain-derived neurotrophic factor (BDNF) levels that correlated with improvement on standard rehabilitation measures (the Barthel index and a motor-function scale) over several months of observation (Gusev et al., Zhurnal Nevrologii i Psikhiatrii Imeni S.S. Korsakova, 2018, PubMed).
- BDNF and neurotrophic effects. Semax has been reported to increase BDNF and nerve growth factor (NGF) gene expression in the hippocampus and frontal cortex in rat studies (Shadrina et al., Journal of Molecular Neuroscience, 2010, PubMed), and to modulate melanocortin receptor signaling, a proposed mechanism for its neuroprotective effects.
Where the evidence is thin
The human clinical evidence here is more substantial than for most peptides on this list, including trials in a serious clinical population (acute and recovering stroke patients). The important caveat is the same one that applies to Selank: this research is concentrated in the Russian clinical research network that developed the compound, without the same volume of independent Western replication that a compound like tirzepatide has. I'd also flag that stroke recovery is a different research context from the general cognitive-enhancement use Semax is typically sold for online, so even the strongest data here doesn't directly speak to that use case.
The regulatory picture
Semax is not FDA-approved in the United States. It's sold by research chemical vendors strictly for laboratory research, not for human use.
Papa's take
Semax has a real, if geographically concentrated, clinical trial history, more so than most peptides discussed in nootropic circles. What I'd push back on is the casual leap from "studied in stroke recovery in Russian trials" to "cognitive enhancer for healthy people," which are different research questions, and I haven't found the second one backed by the same kind of trial the first one has.
